A similar picture emerges in other areas ranging from the health importance of the emotional environment in early childhood to the need for more socially cohesive communities. Since the mid-1970s, a widely shared view that the determinants of health go well beyond medical care has emerged in the western democracies. What mattered in this newly-formed public health agenda were the more basic determinants of health: the work and physical environment; the genes one inherits; and the style of life one adopts. Yet despite nearly two decades of repeated intellectual efforts to redirect health policy away from curative medicine to more fundamental interventions, the task remains, largely undone. The purpose of this volume is to ask why, and to suggest answers and evidence about the determinants of population health that may help redirect national health policies.It is an unusual volume, in every sense a collaborative effort, growing out of the quarterly seminars and long-term research interests of the population Health Program of the Canadian Institute for Advanced Research (C.I.A.R.). Under the auspices of the C.I.A.R., participants distinguished in the social and the health sciences set out between 1988 and 1993 to probe the links between social hierarchy, the influence of "macroenvironmental" factors on illness patterns, the quality of the "microenvironment," and other such determinants of health. Their findings, reported as occasional papers and subsequently further revised and integrated, constitute separate chapters of this book.
In addition to the specific viewpoints adopted and health fields explored here, the editors have provided both an extended introduction and a conceptual framework for the book that elaborates the scholarly group's philosophical and methodological premises. Though the picture presented of the future is not wholly optimistic, this volume will prove essential to an understanding of the underlying public health issues for the next several decades.

Social Determinants of Health provides these kinds of insights and policy advice. For instance, citing the fact that women are more likely than men to work in insecure jobs, in part because responsibility for family care is gendered, one article proposes that Canada go further in encouraging men to share family responsibilities by providing at least one additional month of parental EI benefits only for fathers. Another article outlines the health consequences of non-standard work hours, contingent work, and job insecurity - an excellent contribution to our understanding of employment and wellbeing.

The field of social inequalities in health continues its vigorous growth in the early years of the 21st century. This volume, following in the footsteps of Vicente Navarro's edited collection The Political Economy of Social Inequalities, is a compilation of recent contributions to the areas of social epidemiology, health disparities, health economics, and health services research. The overarching theme is to describe and explain the ever-growing health inequalities across social class, race, and gender, as well as neighborhood, city, region, country, and continent. The approach of this book is distinctly multi-, trans-, and interdisciplinary: the fields of public health, population health, epidemiology, economics, sociology, political science, philosophy, medicine, and history are all represented here.Part I, on social policy, includes Navarro's critique of Sen's influential Development As Freedom, Sen's own analysis of gender and development, a comparison of the consequences of Swedish and British labor market policies, and several analyses of the evolution of international economic inequalities. Part II centers on the contested concept of globalization, with an international empirical analysis of its consequences for global well-being over the last two decades, a description of growing health inequalities by income in the United States since the 1960s, and an account of the expansion of managed care in semiperipheral countries. Part III, on health policy, presents a critique of a controversial 2000 WHO report and two analyses of contemporary U.S. health policy. Part IV, on health care, provides international empirical evidence on the negative effects of privatization, in particular in hospitals, nursing homes, and health services utilization.
