Social Determinants of Health, 2nd Edition gives an authoritative overview of the social and economic factors which are known to be the most powerful determinants of population health in modern societies. Written by acknowledged experts in each field, it provides accessible summaries of the scientific justification for isolating different aspects of social and economic life as the primary determinants of a population's health. The new edition takes account of the most recent research and also includes additional chapters on ethnicity and health, sexual behaviours, the elderly, housing and neighbourhoods. Recognition of the power of socioeconomic factors as determinants of health came initially from research on health inequalities. This has led to a view of health as not simply about individual behaviour or exposure to risk, but how the socially and economically structured way of life of a population shapes its health. Thus exercise and accidents are as much about a society's transport system as about individual decisions; and the nation's diet involves agriculture, food manufacture, retailing, and personal incomes as much as individual choice. But a major new element in the picture we have developed is the importance of the social, or psycho-social, environment to health. For example, health in the workplace for most employees - certainly for office workers - is less a matter of exposure to physical health hazards as of the social environment, of how supportive it is, whether people have control over their work, whether their jobs are secure. A similar picture emerges in other areas ranging from the health importance of the emotional environment in early childhood to the need for more socially cohesive communities. Social Determinants of Health should be read by those interested in the wellbeing of modern societies. It is a must for public health professionals, for health promotion specialists, and for people working in the many fields of public policy which we now know make such an important contribution to health.

Marmot and Wilkinson do a thorough, comprehensive job of exploring the emerging field of how social inequalities affect the health of populations. This is a great resource both for public health professionals and for those who are looking for an accessible way to be introduced to the topic. The second chapter alone - a detailed description of the physiological mechanisms by which psychosocial stress is translated to worse health outcomes - is worth the price. Marmot and Wilkinson's work has significance far beyond the realm of public health, and should be read by policy-makers of all ideological stripes.

      Since the mid-1970s, a widely shared view that the determinants of health go well beyond medical care has emerged in the western democracies. What mattered in this newly-formed public health agenda were the more basic determinants of health: the work and physical environment; the genes one inherits; and the style of life one adopts. Yet despite nearly two decades of repeated intellectual efforts to redirect health policy away from curative medicine to more fundamental interventions, the task remains, largely undone. The purpose of this volume is to ask why, and to suggest answers and evidence about the determinants of population health that may help redirect national health policies.

      It is an unusual volume, in every sense a collaborative effort, growing out of the quarterly seminars and long-term research interests of the population Health Program of the Canadian Institute for Advanced Research (C.I.A.R.). Under the auspices of the C.I.A.R., participants distinguished in the social and the health sciences set out between 1988 and 1993 to probe the links between social hierarchy, the influence of "macroenvironmental" factors on illness patterns, the quality of the "microenvironment," and other such determinants of health. Their findings, reported as occasional papers and subsequently further revised and integrated, constitute separate chapters of this book.

      In addition to the specific viewpoints adopted and health fields explored here, the editors have provided both an extended introduction and a conceptual framework for the book that elaborates the scholarly group's philosophical and methodological premises. Though the picture presented of the future is not wholly optimistic, this volume will prove essential to an understanding of the underlying public health issues for the next several decades.

      In the tradition of Dubos, McKeown, and Geoffrey Rose - but goes further and draws on recent data. By multiple authors. The early chapters make for exhilarating reading. "Producing Health, Consuming Health Care" by Evans and Stoddard in particular will be heavily cited for years to come. Later chapters, on implications for policy, come as a bit of a let down. Ultimately, the book is at its strongest in its account of what produces health but isn't as satisfying in its discussions of repercussions for practice.

      As a policy analyst working on employment-related issues, I look for research and studies that do the following:
      ·provide insights into the problems confronting the labour market,
      ·discuss evidence-based policy changes that will address the problems,
      ·supply specific recommendations for changes to actual existing laws and policies (since policy makers are more likely to react to these kinds of recommendations than to more broad types of recommendations), and
      ·understand that promoting better public awareness of the issues is an important component of policy change (since policy change often happens in response to public pressure).

      Social Determinants of Health provides these kinds of insights and policy advice. For instance, citing the fact that women are more likely than men to work in insecure jobs, in part because responsibility for family care is gendered, one article proposes that Canada go further in encouraging men to share family responsibilities by providing at least one additional month of parental EI benefits only for fathers. Another article outlines the health consequences of non-standard work hours, contingent work, and job insecurity - an excellent contribution to our understanding of employment and wellbeing.

        The field of social inequalities in health continues its vigorous growth in the early years of the 21st century. This volume, following in the footsteps of Vicente Navarro's edited collection The Political Economy of Social Inequalities, is a compilation of recent contributions to the areas of social epidemiology, health disparities, health economics, and health services research. The overarching theme is to describe and explain the ever-growing health inequalities across social class, race, and gender, as well as neighborhood, city, region, country, and continent. The approach of this book is distinctly multi-, trans-, and interdisciplinary: the fields of public health, population health, epidemiology, economics, sociology, political science, philosophy, medicine, and history are all represented here.

        Part I, on social policy, includes Navarro's critique of Sen's influential Development As Freedom, Sen's own analysis of gender and development, a comparison of the consequences of Swedish and British labor market policies, and several analyses of the evolution of international economic inequalities. Part II centers on the contested concept of globalization, with an international empirical analysis of its consequences for global well-being over the last two decades, a description of growing health inequalities by income in the United States since the 1960s, and an account of the expansion of managed care in semiperipheral countries. Part III, on health policy, presents a critique of a controversial 2000 WHO report and two analyses of contemporary U.S. health policy. Part IV, on health care, provides international empirical evidence on the negative effects of privatization, in particular in hospitals, nursing homes, and health services utilization. Part V focuses on occupational health and labor unions, including the crucial role of unions in protecting worker's safety and health. One chapter tells the story of New York's legendary SEIU 1199; another addresses the neglected area of women's occupational health; another provides dramatic case studies on violations of workers' freedom of association and their consequences. Part VI, on social capital versus class, gender, and race, deals with one of the most heated theoretical and empirical debates in contemporary social epidemiology. This problem is of crucial interest to the climate policy community, which has been buffeted by claims and counter-claims concerning the impact of climate change on malaria.

          This important volume examines the contextual determinants of malaria and attempts to develop methods for incorporating them into projections of future incidence. Internationally renowned health specialists, economists, and other social scientists provide regional and global perspectives on risk modeling, the history of eradication efforts, current determinants (including environmental, social, and economic factors), and prospects for new vaccines and drugs.

          The Contextual Determinants of Malaria argues that an association of climate change with increased malaria incidence will have at least as much to do with aging, poverty, urbanization, and population movement as with a rise in global temperatures.
