An excellent resource for anyone who suffers with pain due to Pelvic Floor Disfunction. I found this book to be very informative. It is written in layman's terms so you don't have to be a medical professional to understand it. PFD is a syndrome that effects millions of people worldwide, many of whom are at their wits end as to what to do to find relief. This book will help them understand what they have and what they can do to help themselves. I would highly recommend it to anyone with PFD as well as their spouses,partners or caregivers.
A Life-Changing Read for Those with Pelvic Pain.......2007-05-29
To all those suffering with CPPS/Prostatitis: You must give yourself the gift of reading this book. It is the scripture of the new paradigm for treating chronic pelvic pain. Most importantly, it was written by someone who had actually suffered for years with pelvic pain and, by using the techniques introduced in the book, freed himself from chronic pain. For an author to be able to say to a reader, "I have been where you are", creates overwhelming trust and compassion that is felt throughout this book.
I write this review with the utmost confidence because I, too, suffered with pelvic pain for several years, and was misdiagnosed with "prostatitis" over and over again, as many men have been for years. When I read this book, I just knew it was the truth, because it spoke so fully to every aspect of my condition, both physically, mentally, and emotionally. I committed myself to doing the protocol described in the book, and attended one of the monthly clinics that the authors run. In short, all of my pelvic pain symptoms have completed abated after following the protocol, and in many ways I feel that my overall health is even better than before my pain started, as so much of the protocol taught in the book possesses benefits far beyond healing pelvic pain.
The truth taught in a Headache in the Pelvis has now become scientifically validated -- two recent studies using the AHIP protocol have been published in the JOURNAL UROLOGY and show symptom improvement results in patients like no other studies in the field. AHIP has also recently been featured in an article in Best Life magazine, written by an experienced journalist who found amazing relief from his pelvic pain by using the techniques found in AHIP.
In years to come I know the field of urology will look back on this book as the most groundbreaking treatise on how to help patients find relief from all of the pelvic pain conditions. Groundbreaking, revolutionary, visionary!.......2007-05-25
This expanded edition is superb! Although mainly aimed at a male audience suffering chronic prostatitis and chronic pelvic pain syndrome, this edition includes information relevant to females as well. In summary, this 4th edition adds 63 pages to the 3rd edition. It expands the illustrated discussion of trigger point release. It discusses developments in the home treatment and self-trigger point release program. There are more stories of patients who have undergone the Stanford Protocol. There is a discussion of neurogenic inflammation of the bladder, anxiety, inflammation and interstitial cystitis. There is discussion of other conditions of mucosal inflammation paralleling inflammation of the bladder, the healing of the bladder, the usefulness of cystoscopy. The book contains more illustrations of the location of trigger points and offers a comprehensive illustrated discussion of female pelvic floor physical therapy including locations of female pelvic pain related trigger points, discusses pregnancy and pelvic pain. Lastly, this edition investigates the relationship between emotions and pelvic pain, and other related topics.
Sadly, Amazon has "lost" all the glowing comments and 5-star reviews given to the last edition due to a glitch in Amazon's system. I've copied some of the comments on the last edition of this book for your information. Highly recommended!

Their description of this very private pain was DEAD ON. Their analogy of the pain and the cause is DEAD ON. These authors understand my symptoms and one of the authors has suffered them at least as long as I have. If you have Prostititis you would probably travel miles just to hear or read about someone who can simply describe what you are GOING THROUGH!! My doctor didn`t know what it was or what treatment were effective either and I was just given meds to take daily just because. And he was the GOOD GOOD Doctor! One Urologist charged me $500 to tell me that he could not afford to treat me. So far this has been the only ray of hope for me and the pain is leaving. Hey guys it even explains why you have been curled up in fetal position in your bed crying totally uninteresting in going outside, smiling, talking, moving and cursing sex in agony or masturbating furiously for a moment of release or cursing the idea of life because a demon as been eating at your privates 24/7 365 secretly for a decade. Why does stress and depression seem to make your symptoms worse? Why have you been withdrawing? It is all here and WOMEN ARE WELCOME HERE. You are discussed also. This is no way to live. A Headache in the Pelvis which describes the new treatment for pelvic pain developed at Stanford University is the best book anybody could read who is suffering from pelvic pain. I have read all the editions and just finished the 4th edition which includes treatment for women as well as men. Wise and Anderson are the pioneers in this treatment. I can't praise this book enough!!!

Just a quick comment: It is too bad that this book focuses primarily on males. Female pelvic floor problems are also described and occasionally dealt with, but it would be nice if future editions of this book contained diagrams or chapters geared toward women. Women suffering with these problems really need to know about these techniques too.

Friends, I suffered from Prostadynia over 10 years ago ( 24 age) and it lasted for two years. The pain was mostly in my right teste and it was very bothersome. I went to a few urologists and they ran the gamet of many different tests on me. In the end, I was told that i had no infection and that it was non-bacterial prostititus. One even told me I had a cyst on the right testicle. Alot of medical talk with no answers. Eventually I started to heal my self using exercise, biking, and just relaxation. I made the effort to heal my self through my own ways and experimentation Mind you my pain use to be so bad that I would be in tears sometimes. But i made myself get through it. This last year, I went through a bunch of bad stuff in my life and the pain came back slightly but enough to bother me. I was so upset and knew i had to relax. However, this time around, my life problems were so different and I felt that I did not have the right tools to deal with my pain. Enter this book. About a week ago, I picked up this book. All i have to say is WOW! MAny times while reading it, I shake my head and go "yup" i know that feeling. It's been about a week now, I have noticed about a 50% cut in my groin pain. The exercises help ALOT and so do the relaxation methods. Breathing, accepting the tension, allowing your body to communicate with you...these are very crucial in getting better. Look, if your going to drop money on Doctors to keep examining you or are contemplating suregery. Give this book a chance.
I found this book to be very useful and great. There is hope. Trust me. This book helped me again and if anything is a good read.

These conditions, which the authors refer to as a "headache in the pelvis", have often been routinely misdiagnosed and ineffectively treated, and have affected millions of men and women from all corners of this good globe. This is a book that describes a method to treat this condition for which the authors have published far better results than conventional treatments. The protocol of "A Headache in the Pelvis" was developed by these Stanford researchers, one of whom suffered from pelvic pain for many years and this method came out of his search to finally help himself. It is very unusual for those in the healing profession to offer a treatment that comes from the ancient injunction for the physician to heal thyself. And this is what makes the book so compelling. The book describes this method in an easy to understand and well outlined format suitable for the least sophisticated reader. It is organized with easily referenced sections that take the reader, step by step, through the origins of the condition, the history of other treatments and finally to details of the treatment. If the journey of healing pelvic pain is a thousand miles long, and begins with a single step, then buying this book is clearly the first.

Mind/body medicine at its best, May 30, 2006

This is an inspiring book. As a psychiatrist and physician educator interested in the body/mind connection, I have been looking for clinical applications of meditation and awareness training in the field of medicine. Despite very sophisticated studies proving the benefits of meditation and relaxation exercises, very few clinicians seem to be writing about their direct work with patients. This book is an inspiring example of how these new principles can be used for treating chronic pelvic pain syndromes. The success rate reported in the medical literature of these Stanford researchers is very exciting. I was very appreciative of the author's personal disclosure. As healers and clinicians heal themselves, they bring a special sensitivity to their work. These authors' sensitivity and respect for the deep suffering in their clients' lives pervades the book, and makes it unique among mind-body medicine books. I enthusiastically recommend this book for caring clinicians as well as people in pain looking for relief from their suffering.
Unhelpful Freudian approach.......2007-05-24
This book is full of UN-helpful ideas, like pain is caused by being uptight, by sexual shame, or by unfufillment. It is very old-fashioned and unscientific, like it was written by a "quack." Who cares what came first - pain or depression? YOU know you have a physical pain that needs to be addressed. If you want to try trigger point therapy, look for a better book than this one (Clair Davies is good). If you want to try relaxation techniques, look for a better book or take a class. If you want to try medication, I found major relief with the prescription Effexor. This book is not at all helpful.

From this book I learned much about the pharmacology of pain management. There are remedies for chronic pain, and Dr. Cochran provides examples of his practice to show that there is hope. The book also indirectly asks the reader, especially the person in pain, to examine themselves (their pain and health) along the way.
While the book took me through the trials of both patient and doctor, I found the most useful insights of the book concerned, perhaps ironically, the idea of wellness. In particular, Dr. Cochran writes about a state of intense engagement and satisfaction, which he calls "type two mania"-- in opposition to the mania associated with manic-depression. For a doctor to have a clear idea of health no doubt makes his choice of treatments much more profound and effective than that of those who simply fear that patients will fall into a pattern of abuse and deterioration. Dr. Cochran believes people will try to do good things, provided they are relatively free of pain.
The mania of wellness is a delicate thing. For many the conditions of their lives do not allow them the ability to pursue the intense pleasures associated with work dedicated to quality and lives given to excellence. For others it is the conditions of their bodies that bring their efforts to painful, unfinished ends. For many, a combination of unfortunate circumstances and crippling pain makes the type of health promoted and practiced by Dr. Cochran difficult, if not impossible, to realize.
Dr. Cochran works like a Freudian trained in neurophysiology. In doing so, he discovers patterns of abuse, often forgotten or repressed, that partially account for the suffering of his patients. Pain, in Dr. Cochran's model, is situated in the brain. The brain creates and sustains pain to the point that it becomes a pattern. Pain medication breaks the patterns and provides relief. In many cases, the neuropsychological causes cannot be found or addressed completely. Dr. Cochran honestly reveals his difficulties. His narratives frequently trail off at such points, and the reader can hear, in the silences, the doctor wondering.
Of a piece with these lost narratives is the story of the social conditions of health care in America itself. Therapeutic individualism can only partially mask the fact that 40 million people are without health insurance in the richest country in the world. How many people rely on the tranquilizers of common people: cigarettes, booze and God? (In keeping with his narrative style in which difficult questions are at times asked but unanswered, Dr. Cochran references God in the concluding pages-- an oblique and haunting reference, I think, to the fact that God has been and continues to be the only available opiate for those who suffer without access to proper medications).
This criticism does not mean to detract from Dr. Cochran's work. In fact, history shows that what becomes normal for the rich, one day becomes the custom of the ordinary for good and bad. But it does not come easy.
I certainly recommend this book to chronic pain sufferers, social scientists interested in the practice of medicine, and to physicians who struggle with the delicate issue of prescribing controlled substances.
Sick and Tired of Being Sick and Tired?.......2006-04-28
If you are sick and tired of being sick and tired, please read Dr. Robert T. Cochran Jr's book, Understanding Chronic Pain: A Doctor Talks To His Patients. I am not a medical professional; I am a 51 year old woman who has, for the past 20 years been diagnosed with painful and exhausting illnesses like Chronic Fatigue Syndrome, the Epstein Barr virus, Fibromyalgia, degenerative disc disease that required surgery, menstrual problems, and rheumatoid arthritis. As soon as I find a way to ease the pain of one problem, another excruciating painful problem manifests. Some of you may suffer so badly that you can't hold a job. Your family and friends have had to deal with your unpredictable health, so your social life and intimate connections have dwindled to nothing. Unless your employer is incredibly flexible, you either can't hold a job or have to function under the stress of constant pain, worrying about when you'll make the mistake that will get you fired. To top it off, you really don't care because if you are out of work, at least you will be able to get enough rest.
Worst of all, you may think that this is all in your mind.
Let Dr. Cochran explain to you that it is all in your mind - and in your body, too. After reading this book, you will, for the first time, understand why you hurt and why you can never seem to stop the cycle of pain. You will have information that you can share with your physician that may lead to the path of a life that is not ruled by pain. I have not found the solution to my chronic pain yet, but Dr. Cochran's book has given me something that I have not had for many, many years. It has given me hope. Let it give you hope, too.
Outstanding, original, and compassionate help.......2006-02-26
This is that rare book that will help and inform physicians and patients alike through fascinating and instructive "stories" (case reports.) There are no claims of dramatic breakthroughs but rather a believable success rate that can be raised from the usual 25% or so to more than 50%, which is dramatic when one considers that Dr. Cochran ends up with referrals that have not responded to any number of treatments. The approach Dr. Cochran uses involves an exceptional amount of listening to the whole history, and a willingness to try various combinations of medications and insightful thinking about factors that may trigger or perpetuate chronic pain. Readers can find other books on the subject (see cross references, Amazon), and the scientifically minded can sample controlled studies such as "Combination Therapy for Neuropathic Pain-Which Drugs, Which Combination, Which Patients", New England Journal of Medicine, 352:13, March 31,2005. As a doctor of the same vintage as Dr. Cochran, I agree that the time has arrived to use the insights of modern immunology, i.e. that the body may attack itself in certain diseases (Lupus, Rheumatoid Arthritis, etc.) in such a way that the "immune" response is counterproductive, analogous to how acute pain may provide protective reactions but chronic pain means that the mechanism has gone awry and is defeating the original purpose. Dr. Cochran describes the likely ways in which this perverted response can occur, and what might work to break the cycle. I have bought an extra copy so I can lend it to willing readers while keeping my original copy readily available.
Connecting all the dots of chronic pain.......2005-04-22
I have suffered chronic pain for over 5+ years and have spent a great deal of time researching the subject. I recently picked up Dr. Cochran's book and spent two days devouring his book. On the third day, I e-mailed him at his web site to explain my chronic pain problems and to my surprise, he contacted me by phone 2 days later!! I was so impressed that I subsequently flew from New Mexico to Nashville to meet with him. Dr. Cochran is a gifted physician who cares deeply for his patients and combined with his gift of listening, his expertise with neuropharmacology and his unsurpassed understanding of chronic pain he was able to come with up with a personal game plan for me. Dr. Cochran's truly understands the mind body connection and the implications that emotions can create havoc with your pain. Dr. Cochran's Book exempilfies "Thinking outside the box". I guarantee that you, the reader will be able to relate to some of his case studies. I highly recommend reading this book. I also suggest that your current or past pain physician has not spoken to some of the issues brought forward by this book. This is necessary reading to those who do suffer from chronic pain. Give a copy to your physician or at the very least discuss the book with them, as well as, your friends and family.
From the general to the specific.......2005-03-09
I am so glad that Dr. Cochran wrote this book. Most books of this ilk tend to forget about the patients; relying mostly on treatments, therapies, etc. The ability of the author to speak in normal english is also somewhat unique.
I would like to recommend A Pained Life, a chronic pain journey, as a complementary book to this one. It takes Dr. Cochran's book one step further by detailing one person's pain journey through the medical maze and personal roller coaster that is chronnic pain.

Book Description
People who suffer from chronic pain are typically found to be more anxious and fearful of pain than those who do not. Recent evidence has shown that the fear itself serves as a mechanism through which chronic pain is maintained over time. Even once the muscle or tissue damage is healed, a fear of further pain can lead to avoidance behaviour, which over time, leads to deconditioning (e.g. decreased mobility, weight gain). This in turn leads to further pain experiences, negative expectancies, and strengthened avoidance. It is the reciprocal relationship between fear and avoidance that is thought to be responsible for maintaining pain behaviour and disability. With fear of pain known to cause significant suffering and functional disability, there is a need for a greater understanding of this condition. This is the first book to explore this topic. It starts by introducing the current theoretical positions regarding pain-related fear and anxiety along with relevant empirical findings. It then provides comprehensive coverage of assessment issues and treatment strategies. Finally, the book suggests further areas for investigation. Pain-related fear and anxiety are now receiving considerable attention, and efficient and effective treatments are fast becoming available. This book will help guide and extend our understanding of a condition that has been shown to be associated with substantial suffering and disability.

Self righteous egghead.......2000-08-11
Well my title pretty much sums my idea of the author of this book...Ok I am a sufferer of Chronic Pain for the last 5 years. I am also a very spiritual person and I know all about the releasing of the situation. I released the situation, I released all the negativity surrounding the causation of my pain and injury. My pain did not go away...it has steadily increased. Dont get me wrong I believe the power of the mind is potent however it is not a cure-all. Most of the time the person is a push-over and very gullible, so when they 'cure' themself, well they never head the pain to begin with, or at least not as much as they deluded themself. This book is just another slap in the face for anybody suffering from chronic pain. It is our fault we are in pain. We must like it, we must have something wrong in our minds. Thats the message of this book. I cant tell you how many self-righteous doctors are out there who say that chronic pain is not real...but it is. When your body is in pain, it is very real especially when you are crying in agony for 5 hours straight and people think "get over it" This book only gives more pain then its worth.
Not what I had expected.......2000-06-22
I was hoping for something more substantial. The authors make a few interesting insights, yet the book lacks a lot, in my opinion. Not much to it.

practical information about chronic pain problems.......2002-08-14
This book was very helpful..especially to people trying to understand the difference in chronic pain such as fibromyalgia or arthris such as rheumatoid. It gives just enough scientific information but also offers practical advice. It was easy to read...and the index was very helpful.
Easy to Understand Understanding Chronic Pain.......2002-07-15
I love the quotes selected from other literature that are found at the beginning of each chapter. They provide food for thought as well as a diversion from factual information.
Diagrams and charts are interspersed and clearly simplify the rhetoric.
The book as a whole is easily understood by someone with no medical training. Yet it appears to provide very logical and sensible points that should be beneficial to medical professionals. It contains a wealth of information from which all of us dealing with chronic pain can derive help.
